Apply by Mail

Useful if you prefer a paper application. Download and complete the official Request for Birth Certificate form, include payment by Visa, Mastercard, personal cheque, or money order in Canadian dollars, and mail it to the Office of the Registrar General in Thunder Bay. This route generally takes longer, since the form is reviewed manually.
